Streamline your center’s referral and intake process with Collaborate’s External Referral Forms feature. Implementing External Referral Forms into your workflow will allow your staff to spend less time on manual data entry, and more time assisting clients. 


The referral forms are configurable to match your center’s existing paper forms. Once the forms have been configured for you, a JotForm link is created. The link to the referral form can be emailed to partner organizations for them to fill out, as well as posted as on your center’s public facing website. This grants ease of access to the referral forms by partner agencies.


Referral Forms that have been filled out by partner organizations are placed into a Referral Queue in Collaborate. Staff can then review these referral forms and create cases directly from the form, with information already pre-populated in the intake. Alternatively, if it is found that the referral is associated with a case that already exists in Collaborate, the form can be attributed to the case and accessed for referencing as needed.


All Referral Form data is reportable via Collaborate’s robust reporting feature.
