Some changes have also been made to Client Intakes.


The following changes have been made to Page 1 of Client Intakes:


The “Does this referral meet CAC acceptance criteria?” field has been moved to be below the newly added Case Number fields.

The “Primary Language” field has been changed to “Preferred Language” and a new Secondary Language field has been added to Inquiry Intakes.

A new section has been added titled “Medical or Special Accomodations”. Within this section is a new field “Does client have any medical or special accommodations”. When selecting “Yes”, a “Medical or special accommodations details” text area appears. Entering text in this field will create an alert at the top of the client's profile. The “Forensic Medical Exam Conducted Prior to Intake?” field from page 2 of Intakes was moved to this section. Also, a new field “Was the Client Screened for Medical Needs?” field has been added to this section. When selecting “Yes”, a new lookup field will appear that reads “Was a Medical Exam Recommended?”. When selecting “No” a text area will appear prompting you to explain why not.

The “Alleged Offender / Associated Persons” section at the bottom of Page 1 has been relabeled as “Alleged Offender”, and the corresponding button has been relabeled “Quick Add Alleged Offenders”.

The Caregiver and Education Information sections have been moved from Page 2 to Page 1 of Intakes.

The “Grade” field has been relabeled to “Grade at Intake”.

The following changes have been made to Page 2 of Client Intakes:


All fields on Page 2 have been condensed into a section called “Additional Information”. Selecting “Yes” in the new “Optional Information?” field will open the optional fields.


The Household Income field has been changed to a lookup field, where you can select the appropriate Income range. This was also changed on Associated Person Intakes.

The following changes have been made to Page 3 of Intakes:


The “Did Client make the initial outcry?” field has been moved from Page 4 to Page 3 beneath allegations.

A new field has been added that reads “Do allegations involve a school, student, or teacher/employee/volunteer of the school?”. This should be used to identify when there are allegations against a school employee, volunteer, student, or if the incident took place on school grounds/at a school event. When “Yes” is selected, a School(s) Involved field appears when multiple schools can be listed, as well as a details text area where details can manually be entered.

The following change has been made to Page 4 of Intakes:


A new “Intake ID” text field will appear when selecting “Yes” in the “Is This Case DCFS Information Only?” field.
