After clicking the CSV or PDF link from the Additional Display Formats section, a box will appear like one of these:

Click the pencil icon to edit the settings for the CSV or PDF Output. When you click the pencil, a popup will appear:

It’s recommended to enable the Headers in First Row? The field so that the output has labels for the data.

For the CSV Output, you can usually leave the other fields at their default settings. If needed, adjust the Escape Character, Record Separator, Field Separator, and Charset settings if you are having issues with the formatting as it appears in the CSV Reader application you are using. If you are having issues, consult the documentation of your CSV Reader application for the recommended settings to enter into Collaborate.
